Responsibilities of a Case Manager
A Case Manager at ARC Addiction Recovery Centre serves as a centralized point of contact for information flowing between the multidisciplinary treatment team (MDT) and the patients' nominated next of kin and other important stakeholders. The role includes pre-admission screening, facilitating the intake and admission of service users, and conducting service user orientation. The Case Manager also facilitates meetings with family members to acquire collateral information, manages schedules of the treatment team, and handles all communications with the families or next of kin of the patient.
Pre-Admission and Intake
The pre-admission process involves screening potential service users to determine their eligibility for the program. The Case Manager assists with the intake and admission process, ensuring all necessary documentation is completed. This includes obtaining informed consent on behalf of the facility and consent to disclose information from patients. The Case Manager also manages the 'hospitality concerns meeting' with patients, addressing any issues they may have regarding their stay at the Centre.
Communication with Families
Effective communication with families is a key responsibility of the Case Manager. They facilitate meetings with family members to gather collateral information and report via the Centre's Communication Tracking System (CTS). This ensures that all relevant information is documented and shared with the treatment team. The Case Manager is also responsible for scheduling service user phone call lists and managing any family or supporter complaints, ensuring that families are kept informed and involved in the patient's recovery process.
Coordination with Treatment Team
The Case Manager plays a crucial role in coordinating the efforts of the treatment team. They manage the schedules of the team members on the CTS, ensuring that all appointments and meetings are organized efficiently. By limiting the administrative burden on the psychologists and reducing the risk of information asymmetry, the Case Manager increases the probability that all relevant members of the MDT will have access to pertinent information. This coordination is vital for providing holistic and effective care to the patients.
Documentation and Reporting
Documenting and consolidating information is another important aspect of the Case Manager's role. They are responsible for recording all assessments, reports, and confidential information received from the MDT or family members in the patient's admin file or handover book.
